Who wins and why?

I'd just say Ratchet, he is/was the original and First Aid is/was a pacifist...does not make for high points in the Survival stakes IMHO. Also Ratchet had a sense of humour, a trait called for in his line of work

ED: Oh and Fixit is First Aid in miniature...

Ratchet. If he wasn't killed in the Shuttle he coulda saved Prime on his death bed.

Its not possible to save someone from fatal wounds. Not even Ratchet's expertise could have done it

I dont remember first aid even having a weapon except as toy,, and ratchet had many,, RAtchet wins due to puss out on the side of first aid :-)

i voted first Aid. I liked Ratched,he was the only one who i was really sad about from dieing in the movie. But i feel sorry for for First aid, He aint got any votes.

First Aid. Sure, ratchet has the charisma, the humour et all, but let's face it, that aint gonna land zilch when you oponent is part of a gestalt, these guys never operate on their own, so there's bound to be some protectobot backup

First Aid, cause with Blades on his team he aint gonna be fighting clean, plus u would expect Defensor to make an entrance at some point (only using one arm though so that first aid is the only one fighting

And following this wonderful "they would win the fight, but sadly weren't mentioned" logic, I'm gonna state that Star Saber would cane the lot of 'em. In fact, if I get bored, i might come back later and list every fictional character who could beat the 3 mentioned Autobot medics into an oily pulp...

First Aid may be part of a Gestalt team, but in certain episodes he's been out on his own. He didn't last above 5 minutes where as Ratchet is considerably more resourceful, even unarmed he can and has defended himself with anything at hand, he uses his intelligence as a weapon. First Aid just says "I don't believe in fighting, I'm a medic not a warrior" and doesn't even make an effort to defend himself. He just stands and takes the shots, it's almost as if he has a deathwish....

Even in terms of technical ability, Ratchet knocks the stripes of First Aid...he was "The best tool and die 'bot on Cybertron" if I recall correctly. No such thing was ever said of First Aid. Even their respective catchphrases say a lot for them:

R: "You break it, I'll remake it."
F.A: "An ounce of maintenance is worth a pound of cure"

Now, if you were an injured Autobot whose hands would you prefer to be in?

'Cos TF's mottos are all the be-all and end-all of their characters, aren't they? All those Cons boasting they could be the s**t out of things, and it was the Autobots who went on about peace and love who always caned them...

First Aid had a weapon (and used it) in "Rebirth part 3", and I'm pretty sure he shows up firing away in "Starscream's Ghost" as well.

Either way, the triple threat victory in the end goes to Fixit. He's one bad mutha.
